Notice of Risk / Acknowledgement of Possibility of Injury:
Vaughan Jiu-Jitsu Academy (VBJJ) recognizes their obligation to make the participants and their parents aware of the risks and hazards associated with the activities that are included in the camp. Children/Minors may suffer injuries, possibly minor, serious, or catastrophic in nature. Parents should make their children aware of the possibility of injury and encourage their children to follow all the safety rules and instructions.
VBJJ will not accept responsibility for injuries sustained by any child/minor during the course of activities.
By signing this form, I acknowledge these risks of injury, and I agree that VBJJ or any of their
employees, will not be responsible for any such injuries or illness.
Release and Waiver of Liability:
With the above in mind, and being fully aware of the risks and possibility of injury involved, I consent to have my minor/child participate in the training offered by VBJJ. I, my executors or other representatives, waive and release all rights and claims for injuries or damages of any kind that I or my minor/child may have against VBJJ arising out of participation in the above stated activities.
Regarding illness, should your child or any member living in the family home have, or have been in contact with any individual with the following symptoms: fever, rash, coughing, sneezing, sore throat, pink eye, temperature, you are to refrain from sending your child to joining the camp and to notify VBJJ as soon as possible.
Parents/Guardians are responsible for screening their child(ren) prior to attending the camp symptoms using the Government of Ontario Self-assessment screening tool. Please notify VBJJ by email at vaughan.bjj@gmail.com or call if the child is exhibiting symptoms and unable to attend. Should your child fall ill during practice, the child will be expected to sit out of practice and parent/ guardian will be notified to pick up the child immediately.
Photo Release
I hereby grant permission to VBJJ to use my child’s or children’s photographs on its website, social media or in other official printed publications without further consideration.
I acknowledge VBJJ has the right to crop or treat the photograph(s) at its own discretion.
VBJJ will not post any individual personal information. I agree to indemnify and hold harmless VBJJ from any claims arising out of the use of photographs.
VBJJ reserves the right to discontinue use of any photographs without notice.
THIS RELEASE IS VALID FOR THE DATES OF 03/10/2025 UNTIL 03/14/2025
I, FOR MYSELF, MY SPOUSE, AND CHILD/WARD, HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK.
AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT WE HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
BY SIGNING THIS WAIVER, I CONSENT TO HAVE MY INFORMATION ADDED TO THE INTERNAL PARENT AND FAMILY MAILING LIST.
